Parental Consent

GENERAL PERMISSION: As the parent or legal guardian of my child, I hereby consent for my child to attend and participate in all activities provided by FLOW Summer Camp, sponsored by Grace Church and Haeun Church for the period of June 27 to 29. I also agree to indemnify and hold FLOW Summer Camp, its staff/volunteers, and Korean Community Services harmless and release them from any and all liability for any injury which may be suffered by the registered child, arising out of or in any way connected with participation of FLOW Summer Camp. PHOTOGRAPHY: I understand that pictures and videos could be taken of children participating in various activities during FLOW Summer Camp and would only be used for publicity purposes. ILLNESS: If my child contracts an illness or disease at FLOW Summer Camp, including but not limited to the flu, common cold, RSV, and COVID-19, I will not hold FLOW Summer Camp liable for any medical expenses, personal inconveniences, and other related situations that may arise.
